During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated and completed on 4/28/2025 with a local agency in response to a complaint received by the licensing office on 4/18/2025 relating to a serious injury. The inspector reviewed 1 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/6/2025. A POC submitted after this date will not appear on the public website.
Violations
1On 4/15/2025 Child #1 (Age 9 months) sustained a head injury with visible swelling. Child #1 was placed in the crib to sleep approximately 15 to 20 minutes after the injury. After being notified of the injury and that the child was napping, the parent requested staff wake the child up. Staff #1 and Staff #2 attempted to wake up Child #1 for approximately five to 10 minutes by splashing water on the child's face, wiping the child's nose, and loudly speaking to the child, however the child remained unresponsive throughout this time. Staff #1 and Staff #2 both became concerned when the child wasn't waking up, however, no one called 911. Child #1 was later diagnosed with a concussion.
