During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 6/9/25. The on-site inspection began at 11:00am and ended at 12:30pm.The inspector reviewed compliance in the areas listed above. There were 8 child present and 2 caregivers; the point total was 15.The inspector reviewed 5 children's records and 4 caregiver/household member records on-site. This inspection included document review, interviews, observations and a tour of the facility. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The record for caregiver 1, employed for 1 year, did not contain the results of a central registry check from one state.
Plan of Correction: The provider responded with the following: The out-of-state background check was requested at the time of hire for caregiver 1; however, the results were never received. The provider contacted the state agency on the day of the inspection to follow up on the request and the results of the background check were emailed to the provider. The results have been placed in caregiver 1's record and forwarded to the licensing inspector.
