Inspector Notes
A monitoring inspection was initiated and concluded on 4/20/2022. There were 29 children present, ranging in ages from 6 months to 5 years old, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
3Evidence:
1. Staff #2 (doh: 4/4/2022) did not have a tuberculosis screening on file during the time of the inspection.
Evidence:
1. Peeling and chipped paint was observed in the Preschool classroom (multiple areas on walls by carpet areas), in the Toddler classroom (the black strip underneath the sink cabinet), and in the cafeteria (blue paint on the wall near the cabinets).
2. The cafeteria floors had multiple food stains on the tiles throughout the room.
Evidence:
1. Staff #1 (doh: 4/4/2022) did not have documentation on file of two or more references as to character, reputation, and competency being checked prior to employment.
2. The Director confirmed that they have not checked 2 out of 2 required references for Staff #1 as of the time of the inspection.
