Inspector Notes

A monitoring inspection was initiated and concluded on 4/20/2022. There were 29 children present, ranging in ages from 6 months to 5 years old, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-160-A
Based on a review of staff records, 1 of 2 staff members did not have a TB screening submitted at the time of employment and prior to coming into contact with children.

Evidence:
1. Staff #2 (doh: 4/4/2022) did not have a tuberculosis screening on file during the time of the inspection.
Plan of Correction: We will have the staff member get a TB screening done as soon as possible.
Standard 8VAC20-780-270-A
Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. Peeling and chipped paint was observed in the Preschool classroom (multiple areas on walls by carpet areas), in the Toddler classroom (the black strip underneath the sink cabinet), and in the cafeteria (blue paint on the wall near the cabinets).
2. The cafeteria floors had multiple food stains on the tiles throughout the room.
Plan of Correction: We will get the areas with peeling paint corrected by painting them and we will have the cafeteria floors cleaned and sealed.
Standard 8VAC20-780-70
Based on a review of staff records and staff interview, 1 of 2 staff records did not have documentation that two or more references as to character and reputation as well as competency were checked before employment.
Evidence:
1. Staff #1 (doh: 4/4/2022) did not have documentation on file of two or more references as to character, reputation, and competency being checked prior to employment.
2. The Director confirmed that they have not checked 2 out of 2 required references for Staff #1 as of the time of the inspection.
Plan of Correction: We will ask the staff member for references today and conduct the required references.
