Inspector Notes

Today an unannounced Monitoring Inspection took place between the hours of 10:15am-11:10am. There was a total of 25 children with 8 staff within the supervision guidelines. The children were observed doing an art project, reading books and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation during the inspection. Standard 22.1-289.035-B-2
Based on record review, the center did not ensure to obtain fingerprint-based criminal history check determination letter prior to first day of employment for one staff member.
Evidence:
1) Staff #2 (doh: 2/13/2023) had a fingerprint results determination letter on file that was dated 2/21/2023.
Plan of Correction: We will complete the fingerprints by day one of employment moving forward.
Standard 8VAC20-780-180-B
Based on observation and staff interview, one staff member who works directly with children was not capable of communicating with emergency personnel.

Evidence:
1. Staff #2 (doh: 2/13/2023) was unable to demonstrate the ability to communicate with emergency personnel, if
necessary.
Plan of Correction: We will have the staff member take English classes.
Standard 8VAC20-780-270-A
Based on observation, areas and equipment of the center inside were not maintained in a safe and operable condition.

Evidence:
1. The half door leading into the Toddler/Two's classroom was not working properly during the time of the inspection. The door was dragging on the ground, not locking properly, and had peeling paint on the bottom of the door.
Plan of Correction: We will have a contractor fix the door.

Based on observation, staff did not maintain the required staff to child ratios in the Toddler/Two's room.

Evidence:
1. At approximately 10:28am, it was observed that there was 1 staff member in the toddlers/twos (ages 18 months-2 years old) classroom with 8 children, until approximately 10:31am, when a second staff member came into the classroom. The classroom was out of ratio for approximately 3 minutes.
Plan of Correction: The staff to child ratio was corrected during the inspection. We will talk to the staff and remind them of the required ratios.
