Inspector Notes
Today an unannounced Monitoring Inspection took place between the hours of 10:15am-11:10am. There was a total of 25 children with 8 staff within the supervision guidelines. The children were observed doing an art project, reading books and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation during the inspection. 4Evidence:
1) Staff #2 (doh: 2/13/2023) had a fingerprint results determination letter on file that was dated 2/21/2023.
Evidence:
1. Staff #2 (doh: 2/13/2023) was unable to demonstrate the ability to communicate with emergency personnel, if
necessary.
Evidence:
1. The half door leading into the Toddler/Two's classroom was not working properly during the time of the inspection. The door was dragging on the ground, not locking properly, and had peeling paint on the bottom of the door.
Evidence:
1. At approximately 10:28am, it was observed that there was 1 staff member in the toddlers/twos (ages 18 months-2 years old) classroom with 8 children, until approximately 10:31am, when a second staff member came into the classroom. The classroom was out of ratio for approximately 3 minutes.
