Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 1/16/2025.
The on-site inspection began at 1:35pm and ended at 2:40pm. The inspector reviewed compliance in the areas listed above. There were 22 children present and 5 staff. The inspector reviewed 5 children?s records and 4 staff records [on-site on 1/16/2025.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, are required to be maintained in a clean, safe and operable condition.

In the Toddler classroom, there was a white cabinet under the sink area that was broken. One hinge on the left of the cabinet door was completely detached from the side of the cabinet, causing the cabinet door to fall off and hang.
Plan of Correction: We will repair the cabinet door asap and plan to complete by next Wednesday.

Children under two years of age should not use pillows and filled comforters.

Child #2 was observed sleeping in a crib with a blanket in the crib with the child.
Plan of Correction: The blanket was removed from the crib during the inspection. We will not use blankets for children under 2.
Standard 8VAC20-780-60-A
The center shall maintain and keep a separate record for each child at the center that contains 2 emergency contacts with name, address and phone number.

Child #5 was missing the name, address and phone number for 1 of 2 required emergency contacts.
Plan of Correction: We will ask the parents for the second emergency contact today.
