The ratio for infants from birth up to 16 months is 1:4 (staff:children).
Staff #1 was observed standing in the open gate connecting the infant and toddler classrooms to supervise all children. There was a total of 5 children, including 2 infants, which require a 1:4 ratio at all times. All children were within sight and sound supervision, however, the ratio for the youngest age group was not being followed.

Plan of Correction: We will make sure it won't happen again. Talked to staff made it clear about 1:4 ratio.

Pillows and filled comforters are not be used by children under two years of age.
Infant #1 was observed to be sleeping in a crib with a blanket completely covering the infant from head to toe.

Plan of Correction: We will give training to staff about safe sleep. No more blanket will be used, only zip up sack.
