Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 11:05am and 12pm. There were 11 children directly supervised by 2 caregivers. The home, 2 caregiver records, 3 children?s records, 4 household member?s records, evacuation drills, and emergency supplies were inspected. Children were observed playing outside. Activities were well-organized and caregivers provided encouragement and descriptive praise often. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-800-100-A
Based on record review, the provider did not obtain documentation of a physical examination within 30 days after the first day of attendance for 1 child.

Evidence:
1. The physical in the record for Child #2 (start date 10/4/2022) was dated 11/9/2022.
Plan of Correction: Physical exams will be obtained within 30 days after the child's first day.
Standard 8VAC20-800-70-B
Based on record review and provider interview, the provider did not obtain the parent's written acknowledgement of the receipt of information required in 8VAC20-800-(2)-70-A.

Evidence:
1. The record for Child #1 (start date 1/10/2023) did not contain the Information for Parents form.
2. The provider stated she gave the form to the parents, but they had not returned the signed copy.
Plan of Correction: The form was given to the parent. The provider will ask the parent to return the signed copy.
