Inspector Notes

An unannounced monitoring inspection was conducted on 06/25/2024 between the hours of 9:10am-10:20am. There were 10 children in care directly supervised by the provider and 1 assistant caregiver. The home, 2 caregiver records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing outside. Diapering and handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Based on record review, 1 of 2 caregivers did not complete a sworn statement prior to their first day of employment.

Evidence:
1. Caregiver #2 began employment on 04/03/2024.
2. The sworn statement in the record for Caregiver #2 was dated 04/05/2024.
Plan of Correction: Sworn statements will be obtained prior to first date of hire.
Standard 8VAC20-800-170-A
Based on record review, the provider did not obtain a current report of tuberculosis screening for 1 caregiver at the time of hire.

Evidence:
1. Caregiver #2 began employment on 04/03/2024.
2. The tuberculosis screening results in the record for Caregiver #2 were dated 04/09/2024.
Plan of Correction: Tuberculosis screenings will be obtained prior to first date of hire.

Based on observation, medication was not kept in a locked place using a safe locking
method that prevents access by children.

Evidence:
1. Medication prescribed to Child #5 was not kept in a locked place.
Plan of Correction: The provider will purchase a locked box for medication storage.
