Violations

4
Standard 8VAC20-780-160-C
Based on record review and interview, staff did not submit a subsequent tuberculosis (TB) screening at least every two years from the date of the initial screening.
Evidence: Staff #1's most recent documented TB screening is dated 3/10/2021. Center management confirmed that the staff person had not completed a repeat TB screening.
Plan of Correction: They are aware that it's expired and they were supposed to get it done. I will talk to them about making the appointment.
Standard 8VAC20-780-280-B
Based on observation, hazardous substances such as cleaning materials, were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. There was two bottles of cleaning agents observed on an open shelf in the children's bathroom in Classroom 1.
2. The lock on the cabinet which stores cleaning materials in Classroom 4 does not work.
Plan of Correction: We will fix the lock.
Standard 8VAC20-780-280-G
Hazardous substances that were not kept in the original container did not clearly indicate their contents.
Evidence: There was a unidentified spray bottle observed on the shelf in the children's bathroom in Classroom 1. Center management confirmed that the spray bottle contained a mixed cleaning agent.
Plan of Correction: We'll label the bottles.
Standard 8VAC20-780-500-A
Based on observation, hand washing requirements were not met.
Evidence: The children in Classroom 4 did not have their hands washed with warm water and soap prior to eating their morning snack. The children were observed having hand sanitizer squeezed onto their hands before eating.
Plan of Correction: I'll remind them about hand washing before meals.
