Inspector Notes
An unannounced monitoring inspection was conducted on 2/1/2024 at 1:45pm. There were 47 (19 mos to 5 years) directly supervised by 9 staff. The physical plant, 3 staff records, 5 children?s records, 3 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping, waking up from nap, and dismissal. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations
6Evidence:
1) observed 2 small nails protruding from the stone/window sill in the primary room.
2) observed a crack in the stone wall large enough for children to fit their fingers in the primary room.
3) observed 2 large screws protruding from the fence on the younger playground.
4) observed peeling paint along the lattice/fencing on the playground.
Evidence:
1) observed hand sanitizer accessible to children in all 4 classrooms.
2) observed hand lotion labeled "keep out of reach of children" and Lysol spray in an unlocked cabinet, accessible to children in the 3s classroom.
3) observed "Stress EZ pure essential oils" marked "keep out of reach of children" on the window sill.
4) observed miracle gro, labeled "keep out of reach of children" in an unlocked cabinet, accessible to children.
Evidence: reviewed the facility records and found that there was not an annually updated injury prevention plan.
Evidence: observed a small container of diaper ointment/cream that was not labeled with the child's name.
Evidence: observed a first aid kit in a backpack on a chair in the kindergarten classroom that was accessible children.
Evidence: reviewed 10 injury reports and found that 7 of the reports did not have documentation of the time parents were notified.
