Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 08/09/2021 and concluded on 08/09/2021. The director was contacted by telephone and a virtual inspection was conducted. There were six children present with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
1Evidence: 1) The most recent TB screening in the record for Staff #1, hired on 09/05/2018, was dated 08/05/2018. The TB expired on 08/05/2020. 2) During interview, a member of management confirmed this is the most recent TB screening on file for Staff #1. Staff should
resubmit TB test results at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department.
