Inspector Notes
The licensing inspector conducted an unannounced monitoring inspection on Thursday, September 29, 2022 from10:45am to approximately 1:40pm. There were a total of 62 children in care in the direct care of 12 staff members. During the inspection, the children and staff were
observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility including classrooms, kitchen, hallways, playgrounds, and bathrooms were inspected. The required postings were reviewed and found to be in
compliance. Medication is administered when required and medication and medication authorizations were reviewed. The center is equipped with toys and supplies and items were available to the children. During the inspection, six children?s records and six staff records were
reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
2Evidence: The record for Staff #1, employed 09/10/21, indicated the staff had resided in another state outside of Virginia within the last five years. The record did not contain a search of the child abuse and neglect registry from that state. The out-of-state search for founded complaints of child abuse or neglect is required to be requested within the first 30 days of employment.
Evidence: Five written injury records were reviewed. The following documentation was missing -
1) Injury record #1 did not contain documentation on any future action to prevent reoccurrence of the injury; 2) Injury record #2 did not contain documentation on any future action to prevent reoccurrence of the injury, and a second signature from a staff or parent; 3) Injury record #3 did not contain documentation on any future action to prevent reoccurrence of the injury; 4) Injury record #4 did not contain documentation on any future action to prevent reoccurrence of the injury, and a second signature from a staff or parent; 5) Injury record #5 did not contain documentation on any future action to prevent reoccurrence of the injury, and a second signature from a staff or parent.
The center should maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following: date and time of injury; name of injured child; type and circumstance of the injury; staff present and treatment; date and time when parents were notified; any future action to prevent reoccurrence of the injury; staff and parent signatures or two staff signatures; and documentation on how parent was notified.
