Inspector Notes
An unannounced monitoring inspection was conducted on Thursday, June 27, 2024, to determine the center's compliance with licensing standards. Two inspectors were on site from 11:35am to approximately 1:35pm. There were a total of 32 children present in the direct care of seven staff members. Upon the inspector's arrival, the children and staff were observed in their designated areas of the center. Some of the children were observed during rest time, and others were observed eating lunch. The center is equipped with age-appropriate materials and equipment for the children's use. Staff were engaged with the children and offered guidance when needed. The areas where children receive care were inspected and found to be in compliance. The required postings were observed. Medication is administered and medications and authorizations were reviewed. During the inspection, five children's records and five staff records were reviewed. Information gathered during the inspections determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
2ensure three staff members shall submit documentation of a negative tuberculosis screening within the required time frames.
Evidence: 1. The tuberculosis screening in the record of Staff #2, employed on 06/04/24, was
completed on 03/21/24.
2. The tuberculosis screening in the record of Staff #3, employed on 09/22/23, was completed on 11/12/22.
3. The tuberculosis screening in the record of Staff #4, employed on 06/04/24, was completed on 04/27/24.
4. During interview, a member of management confirmed the tuberculosis screenings for Staff #2, Staff #3, and Staff #4 were completed more than 30 days prior to employment.
Documentation of the screening shall be submitted at the time of employment and prior
to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence: A bottle of disinfectant spray was observed on a shelf in one of the bathrooms used by children in the Gala classroom. A container of disinfectant wipes was observed on a shelf in the second bathroom used by children in the Gala classroom.
