Inspector Notes

An unannounced inspection was initiated on 9/10/2024 and completed on 9/19/2024 in response to a complaint received by the licensing office on 7/15/2024 relating to allegations in the areas of supervision, behavior guidance, forbidden action and injury reporting. The center does not operate during the summer. The inspector reviewed 2 children?s records electronically. This inspection included document review, interviews and observations.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, a violation that was not reported was found related to general record keeping and is documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/26/2024. A POC submitted after this date will not appear on the public website.

Records and reports on children required by this chapter shall be maintained and made accessible for two years after termination of services.

The immunization and physical records for child #2 were not maintained and made accessible for two years. The records were requested approximately 4 months after the last date of attendance and were not available.
