Inspector Notes
An unannounced monitoring inspection was conducted on 11/9/2023 from 8:28 am-9:40 am. There were a total of 8 children (20 points) being supervised by 1 direct caregiver from approximately 8:28 am- 9:04 am. An additional caregiver arrived at approximately 9:04 am through the end of the inspection. The home, 2 caregivers' records, 5 children?s records, 1 household member record, evacuation drills, emergency supplies, and policies were inspected. Children were observed engaging freely, eating breakfast, learning about letters, and writing in their journals. Areas of non-compliance are identified in the Violation Notice.

5Evidence:
1. The findings of the most recent inspection (06/14/2023) of the family day home was not posted.
2. The family day home's most recent license, issued on July 27, 2023, was not posted.
Evidence: There was no documentation of a physical examination completed by or under the direction of a physician prior to or within 30 days of Child #2's first day of attendance.
2. The start date for Child #2 is 6/20/2023.
Evidence:
1. The inspector arrived at the home at approximately 8:28 am and observed 8 children for a total of 20 points with Caregiver #1.
2. Caregiver #1 was alone with the children until Caregiver #2 arrived at approximately 9:04 am.
Evidence: The last annual review of the emergency plan was dated 11/3/2022.
Evidence: There was no documentation that Child #2 had been adequately immunized prior to their first day of attendance, as required by Virginia Code ? 32.1-46 A.
