During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 7/21/2025 and completed on 7/21/2025. The on-site inspection began at 10:30 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 3 children present and 2 caregivers; the point total was 6. The inspector reviewed 3 children?s records and 2 caregiver records on-site. This inspection included document review, a tour of the home, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/25/2025. A POC submitted after this date will not appear on the public website.

The immunizations received shall be signed by a licensed physician, the physician's designee, or an official of a local health department.

The immunization record available for Child #1 was missing a physician's signature, the physician's designee, or an official of a local health department.

Areas of the family day home inside shall be maintained in a safe condition.

A wall next to the bathroom and a wall next to the entrance door have areas of chipped paint.
Plan of Correction: We are renovating. Wall decor was ordered.

A box of toilet cleaner was inside a tub in a bathroom that children use.

A container of air freshener was located in a blue storage box labeled "cleaning supplies" at the bottom of the diaper table.

Child-resistant protective covers larger than 1-1/4 inches in diameter shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

A surge protector located on the floor next to the television is missing two protective covers.

An outlet above the bathroom sink was missing a protective cover.

Pet #1 and Pet #2?s rabies vaccinations have been expired for over four months.
