Inspector Notes

An unannounced, onsite renewal inspection was initiated on 11/14/2024 and completed on 11/19/2024. The on-site inspection began at 12:45 pm and ended at 3:15 pm. The inspector reviewed compliance in the areas listed above. There were 59 children present and 16 staff. The inspector reviewed 5 children?s records, 5 staff records and 5 applicant records.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

There was no documentation of requesting a child abuse and neglect check from a state for Staff 5 who has been employed for more than 2 months.
Plan of Correction: Will request the information.
Standard 8VAC20-770-40-D-1-a
The center is required to obtain a completed sworn statement, a criminal record check, and a central registry search by the end of the 30th day for all board officer/agents.

Agent 2, who has been in office for multiple years, did not have a completed sworn statement Agent 4, who has been in office for multiple years, did not have a completed central registry search.
Plan of Correction: The information will be obtain/requested.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

On 11/14/2024: there were 4 children present in the Toddler A class and there were 6 children marked present on the attendance record; there was not an attendance record for the Infant A or Infant B classes that documented the arrival and departure of each child; and there were 13 children marked present for before school care, however, the children had departed for school and where no longer in care.
Plan of Correction: Will be changing the attendance form to document both arrival and departure.
