Inspector Notes
An unannounced monitoring inspection was initiated on 3/19/2025 and completed on 3/28/2025 in response to a self-report received by the licensing office on 03/14/2025 relating to staffing and supervision. An on-site inspection was conducted on 3/19/2025 with local agencies. The inspection included a tour of the facility, interviews, and observations. The inspector reviewed 1 child record and 6 staff records on-site.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Though not a part of the original self-report, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 04/04/2025. A POC submitted after this date will not appear on the public website.
Violations
3On 3/14/25, Child 1 (22 months old) opened two gates that were closed, leaving the back playground area unsupervised. Child 1 wandered into the side street, then crossed the street, and entered a vacant building that was under construction. Child 1 exited the building and wandered into the middle of the main street (speed limit 35 mph) where multiple cars and a semi-truck were traveling. The child was found by a law enforcement officer and then returned to the center.
Staff 5 and Staff 6 were supervising Child 1 and did not know that the child was missing. Additionally, two staff on the same playground area who were supervising another group of children did not know Child 1 had left the playground. The child had been unsupervised for approximately 9 minutes.
On 3/14/2025, Child 1 (22 months) was unsupervised, left the playground area, and walked into the surrounding neighborhood and streets. Staff did not have sight and sound supervision of Child 1 for approximately 9 minutes.
The center?s playground safety policy states ?do not congregate with other teachers; choose a location with an overall view of the playground; ensure all gates have childproof locks?. On 3/14/2025, four staff were sitting in one central area together on the playground. The childproof lock on the gate was not locked. Child 1 (22 months) left the playground unnoticed by staff.
The center also has a policy that states ?no child is ever to be left alone or unsupervised; teachers should be aware of children in their care at all times?. Child 1 (22 months) exited the playground area and was alone and unsupervised for approximately 9 minutes. Staff was not aware of this child being missing from their care.
