During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 06/9/2025 , as a part of licensure period. The on-site inspection began at 9:35am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were 74 children present and 18 staff. The inspector reviewed 6 children?s records, two medical records and 5 staff records on-site. This inspection included document review, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

In the two?s A and preschool class the cabinet doors lock was broken. This is where cleaning supplies were housed and one for teachers personal items.
In the preschool classroom there was a teacher chair in the class where the seat was ripped open and the foam was exposed. Children were observed in this area.
Plan of Correction: Additional locks were placed on the cubbies until maintenance can fix it.
Chair was removed by the end of the licensing visit.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.

Staff #5, who has been actively working at the center for the past 3 days, did not have any completed reference verifications on file.
