Inspector Notes

An unannounced monitoring inspection was conducted on 8/17/22 from 9:45am to 11:10am with the provider and one assistant. There were seven children in care, ranging in age from 12-months to four-years-old, for a total of 20 points. Children were observed having free play with toys and napping. Six child records, the provider?s record, one assistant record, and one household member record were reviewed. The provider has current certification in CPR and First Aid. The provider reported that there were no medications for children on site, but the provider has current certification in Medication Administration Training (MAT). The emergency drill log, attendance, and menu were also reviewed. Based on review of six child records, the provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance for all children. Evidence: 1. The record for Child A, with a start date of 11/8/2021, did not contain documentation of a physical. 2. The record for Child B, with a start date of 8/6/2018, did not contain documentation of a physical.
Plan of Correction: I will request this from the family today. Moving forward I will be sure to get this within 30 days.
Standard 8VAC20-800-90-A
Based on review of six child records, the provider did not obtain
documentation that a child had been adequately immunized before attending the family day home. Evidence: The immunizations on record for Child A, with a start date of 11/8/2021, were dated 2/2/2022.
Plan of Correction: Moving forward I will be sure to get this before children start.
