Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews with staff.

An unannounced renewal inspection conducted on 8/4/2021. Staff were contacted by telephone and a virtual inspection was conducted. There were 13 children present, with six staff supervising. The inspector reviewed compliance in the areas of administration, staff qualifications and training, staffing and supervision, programming, and special care provisions and emergencies. A total of two child records and three staff records were reviewed and in compliance.

Information gathered during the inspection determined non-compliance with one applicable standard or law and the violation was documented on the violation notice issued to the program. Based on review of five board officer/applicant/agent records, and staff interview, it was determined that no Out of State Criminal History Name Check was requested by the end of the 30th day of becoming a board officer. Evidence: the record for board officer #3, (date of service: 7/1/2021) who indicated that they have lived out of the state of Virginia in the past five years, had no documentation of an out of state criminal history name check request.
Plan of Correction: Staff stated they will request an out of state criminal history name check for board officer #3.
