An unannounced monitoring inspection was initiated and completed on 8-8-25 in response to a self-report received by the licensing office on 8/4/25 relating to an injury requiring outside medical treatment. The inspector reviewed 1 child?s records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
