Inspector Notes
A monitoring inspection was conducted on 4/29/2024 from 9:15 am to 10:51 am. Five children, ages two years to four years (total of 10 points) were in care with two caregivers. The Licensing Inspector reviewed three children?s records, two provider records, two household member records, one pet record, and other program records. The children were observed in free indoor play, exercising to a tape/tv show, and outdoor play.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

The CPR certification training for Staff 1 expired January 2024.
EVIDENCE:
The first aid certification training for Staff 1 expired January 2024.
Based on record review, the family day home failed to ensure that each caregiver and adult household member was screened for TB at least every two years.
EVIDENCE:
1. The most recent TB screening for Staff 2 was dated 9/16/2023.
2. The most recent TB screening for Household member 1 was dated 1/3/2022.
Based on observation, the family day home failed to have a potentially poisonous substance stored away from food in areas inaccessible to children.
EVIDENCE:
A bottle of Windex was on the kitchen counter, accessible to children.
Based on record review, the family day home failed to obtain some required information in children?s records.
EVIDENCE:
The record for Child 1 had no information regarding prior schools or care and no house numbers for either of the emergency contacts.
EVIDENCE:
The first aid kit had no tweezers, and Staff 1 confirmed this, and stated there were none elsewhere in the house.
EVIDENCE:
The radio did not operate after the batteries were installed. This was confirmed by Staff 1, who also stated there were no extra batteries in the house.
Based on document review, the center failed to have monthly evacuation drills.
EVIDENCE:
The family day home?s Evacuation Drill log shows a drill has not been done since August 2023.
