Inspector Notes

An unannounced monitoring inspection was conducted on October 16, 2023 between the hours of 9:30am and 11:10am. There was a total of 32 children with 7 staff members supervising. Children were observed during snack time, engaged in group activities (story time and music), playing on the playground, eating snack, toileting and handwashing. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, children and staff records, special care and emergencies. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard 8VAC20-780-40-G
Based on record review, the center did meet the proof of child identity and age requirements as stated in ? 22.1-289.049 of the Code of Virginia.
Evidence: The record for Child #3 did not have documentation of viewing proof of the child's identity and age within seven (7) days of the date of enrollment (9/18/23).
Plan of Correction: I called the parent to tell her I need the proof of identity when the child is picked up today.
Standard 8VAC20-780-500-A
Based on observation, the staff did not wash their hands with soap and running water before and after helping a child use the toilet.
Evidence:
1. Staff #3 in the 3's class, did not wash her hands before and after assisting two (2) children use the toilet.
2. Staff #4 in the Two's class, did not wash her hands before and after assisting three (3) children use the toilet. The staff used the same set of gloves with two children.

Repeat Violation
Plan of Correction: We will retrain all our staff at our staff meeting tomorrow on proper handwashing and toileting procedures. The Director observed the staff not washing her hands and was corrected immediately. Violation corrected during inspection.
Standard 8VAC20-780-500-B
Based on observation, disposable diapers were not disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Evidence: The foot-operated storage system on the first floor was broken and required a staff member to lift the lid during disposal.
Plan of Correction: I will order a new storage system today.
