Inspector Notes
An unannounced monitoring inspection was conducted on October 16, 2023 between the hours of 9:30am and 11:10am. There was a total of 32 children with 7 staff members supervising. Children were observed during snack time, engaged in group activities (story time and music), playing on the playground, eating snack, toileting and handwashing. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, children and staff records, special care and emergencies. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations
3Evidence: The record for Child #3 did not have documentation of viewing proof of the child's identity and age within seven (7) days of the date of enrollment (9/18/23).
Evidence:
1. Staff #3 in the 3's class, did not wash her hands before and after assisting two (2) children use the toilet.
2. Staff #4 in the Two's class, did not wash her hands before and after assisting three (3) children use the toilet. The staff used the same set of gloves with two children.
Repeat Violation
Evidence: The foot-operated storage system on the first floor was broken and required a staff member to lift the lid during disposal.
