During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on July 7, 2025. The on-site inspection began at 8:15am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. There were 29 children present and five staff. The inspector reviewed four children?s records and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-B
If the child has had a physical examination prior to attendance, it shall be within six months prior to attendance for children aged 19 months through 24 months. The physical examination for Child #2 had been completed over six months prior to first day of attendance.
Plan of Correction: I will ensure immunization/physical records are current and will write the date received to ensure they are up-to-date.
Standard 8VAC20-780-40-G
The center shall meet the proof of child identity and age requirement as stated in the Code of Virginia.

The file for Child #5 did not have proof of identity and the child had been enrolled over three months.
Plan of Correction: I emailed the family stating child's ID is needed before returning. Also, no child will start without ID going forward.
