Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and staff interviews.

A renewal inspection was initiated on 9/30/2021 and concluded on 9/30/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 35 children present, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, staff qualifications and training, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 3 child records and 3 staff and agent records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-185-130-A
Based on record review, the center did not obtain documentation that 2 of 3 children received the immunizations required by the State Board of Health before the children attended the center.

Evidence:
1. Documentation of immunizations in the record for Child #1 (start date 7/6/21) were dated 7/12/21.
2. Documentation of immunizations in the record for Child #2 (start date 8/16/21) were dated 8/19/21
Plan of Correction: Immunizations for Child 1 and 2 were obtained and placed in their files.
Standard 22VAC40-185-260-A
Based on record review and staff interview, a current annual fire inspection report from the appropriate fire official having jurisdiction was not provided.

Evidence:
1. The most recent fire inspection available for review occurred on 09/04/2020.
2. Staff acknowledged an attempt was made to schedule the inspection but it could not be completed due to an outstanding fee due that the center was unaware of.
Plan of Correction: The fee has been paid. Once processed, an inspection will be scheduled.
