Inspector Notes

The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on
5/22/2023 relating to forbidden actions during an incident on 5/17/2022. Interviews were conducted with staff and documents were reviewed.
The information gathered during this investigation does not support the allegation of forbidden action, however the center was cited for two other
violations related to the incident.

Standard 8VAC20-780-50-A
Based on interview, information in children's
records was not treated confidentially.
Evidence:
1. Staff A stated she told Parent B the name of the child that threw a toy and hit Parent B's
child in the mouth.
2. Parent B stated Staff A told her the name of the child that threw a toy and hit Parent B's
child in the mouth.
Plan of Correction: we have reviewed and signed a confidentiality agreement with all staff.
Standard 8VAC20-780-550-P
Based on review of the written record of a child's minor injury, the center did not maintain a written record that included all required information.
Evidence:
1. The accident report dated 5/17/2023 for Child #1 was missing one more staff signature or the parent's signature, the method the parent was notified and date and time when parents were notified.
Plan of Correction: Verify all parts of the accident report to be filled.
