Inspector Notes

An unannounced monitoring inspection was conducted today to ensure compliance with the Intensive Plan of Correction (IPOC) that was entered into with the Department of Education on 6/15/2023.
The facility is not in compliance with the IPOC.
Another violation was cited that was not part of IPOC; however, found to be out of compliance during the inspection.

Licensing Inspectors Marbert and Vaughn were present today from 12:30 PM - 1:25PM.

Standard 8VAC20-780-270-A
Repeat Violation

Based on observation and interviews, areas and equipment inside the center were not maintained in a safe and operable condition.
Evidence:
*One toilet in the boy?s hallway bathroom had a plastic garbage bag covering the toilet because there was no toilet seat installed.
*Peeling paint was observed at the sink in the boy?s hallway bathroom.
*There are no toilet paper holders installed in the girl?s hallway bathroom.
*One toilet in the girl?s hallway bathroom is not securely installed. The entire toilet swiveled sideways when bumped during the inspection.
*There is peeling paint and caulk at the sink in the girl?s hallway bathroom.
*The panels separating the toilets in the girl?s hallway bathroom are not installed securely. The metal panels are hanging loose from the wall where they were screwed into the drywall.

Based on observation and staff statement a ration of 1:4 was not maintained in a room that contained children under the age of 16 months.
Evidence:
The ?older infant? room contained one child that was 14 months old. The appropriate ratio for the room was 1:4. There were 5 children and 1 staff present in the room during rest time.

Based on interviews and observation the licensee failed to demonstrate operational responsibilities to maintain compliance with these standards.
Evidence:
1.On June 15, 2023, the Department accepted an intensive plan of correction
submitted by the licensee which addressed corrective actions implemented by the licensee
for violations pertaining to building maintenance, supervision of children, cribs, cots, rest mats and beds and special feeding needs. On August 21, 2023 an inspection was conducted on the implementation of the plan of correction.
Repeat violations were documented in the area of building maintenance.
