Inspector Notes

A non-mandated inspection, resulting from a self-reported incident that occurred on 3/30/2023, was conducted by licensing staff on 3/30/2023. The inspector reviewed policies and procedures and reviewed video camera footage.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 8VAC20-780-40-E
Based on the review of policies and procedures and video camera footage, the center failed to maintain the operational responsibility of the licensee to ensure that the center?s activities, services and facilities are maintained in compliance with the center?s own policies and procedures that are required by the standards.

Evidence: The center's required lifting of children policy requires children should always be lifted underneath both arms (armpits). Never lift a child by the wrist, one arm or any area other than the armpit. This could easily cause a dislocation or an injury. The center's video recording showed Staff #1 walking to the rug located in the far right corner of the room and grabs Child #1 by the right arm between the elbow and shoulder before lifting Child #1 under the armpits and places Child #1 in the hallway by the door to the classroom. Staff #1 stands at the doorway and then lines up the rest of the children and approximately one minute later steps into the hallway and grabs Child #1 by the right arm between the elbow and shoulder lifting Child #1 off of the hallway floor.
Plan of Correction: Provider will ensure all staff understand and follow written policy for lifting children.

Based on the review of video camera footage, the center failed to ensure that staff shall not roughly handle or shake a child.

The center's video recording showed Staff #1 walking to the rug located in the far right corner of the room and grabs Child #1 by the right arm between the elbow and shoulder before lifting Child #1 under the armpits and places Child #1 in the hallway by the door to the classroom.
