Inspector Notes

An unannounced monitoring inspection was conducted on 1/22/2024. There were 74 children, ages 4 months -5 years, 16 staff and 4 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medications for children were observed during the inspection. A total of 5 children?s records and 3 staff records were reviewed. The children were observed during center time, lunch and naptime. The inspector arrived for the inspection at 10:30am and departed the center at 12:15pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 8VAC20-780-130-A
Based on review of five children?s records, the center failed to ensure that all immunization records were obtained before the child?s first date of attendance.

Evidence: Child #3 first date of attendance was 3/30/2022 and on the day of the inspection did not have the required immunization record.
Plan of Correction: Provider will obtain immunization record documentation from parent.
Standard 8VAC20-780-270-A
Based on observation, the center failed to ensure that areas and equipment, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence: The wall to the left of the white storage cabinet in the Pre K 2 room has three holes approximately ?? in diameter. There were three children from the Pre K 2 room observed playing on a rug on the floor directly in front of the wall.
Plan of Correction: Provider will ensure all three holes are patched.
