Inspector Notes

An unannounced monitoring inspection was conducted today. There were 12 children present (40points)/3 caregivers. The children were observed sleeping, playing with blocks and singing songs with caregivers. The physical plant, menu's, administration, staffing and supervision, attendance logs, evacuation logs, emergency supplies, children records, staff records and household member records were reviewed. Based on observation the provider failed to ensure that potentially poisonous substances shall be stored in areas inaccessible to children.

Evidence:
Diaper ointments labeled keep out of reach of children were in an area accessible to children.
Plan of Correction: Diaper creams and ointments were moved during the inspection.
Standard 8VAC20-800-180-A
Based on review of caregiver records, the provider failed to obtain for each caregiver every two years form the date of the first test or screening.

Evidence:
Caregiver #2's record did not have documentation of a tuberculosis test or screening.
Plan of Correction: I will have caregiver to obtain and submit a tuberculosis screening.
