Inspector Notes

A monitoring inspection was initiated on 10/2/2023 and concluded on 10/2/2023. There were 15 children present with three staff supervising and one kitchen staff present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records were reviewed. The inspection started at 10:05 am and concluded at 11:35 am. Following the inspection a meeting with the new HR person was held from 12:30-3:30 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Based on record reviews the center failed to ensure that Each child shall have a physical examination by or under the direction of a physician, before the child's attendance; or within 30 days after the first day of attendance.
Evidence:
Child #5 (first day 8/21/23) does not have a physical examination on file within 30 days after the first day of attendance.
Plan of Correction: The center director contacted the parent during the inspection to request that the physical be sent in.
Standard 8VAC20-780-510-G
Based on medication reviews the center failed to ensure that medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.
Evidence:
The medication for child #2 was not labeled with the child's name, name of medication, the dosage amount and the time/times to be given.
Plan of Correction: The center director will have the parent provide a prescription label for this medication.
