Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/4/24 and completed on 9/6/24. The on-site inspection began at 11:00 and ended at 12:45. . The inspector reviewed compliance in the areas listed above. There were 9 children present and 10 staff.
The inspector reviewed 4 children?s records on-site and 5 staff records on-site electronically on 9/6/24. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-330-B
Based on observation and measurement, the playground did not have the required depth of resilient surfacing.
Evidence:
On the large playground, there was grass growing through out the use zones of the play equipment. Mulch varied between 1-4 inches around all of the equipment including but not limited to the swings, rockers, the climbing wall, slides and under the rungs.
Plan of Correction: A work order has been placed for more mulch.

Based on observation, the center did not have an operable battery operated radio.
Evidence:
Their was a radio but it would not work with the batteries.
Plan of Correction: A working battery operated radio will be found.
