Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 10/01/2021 and concluded on 10/01/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 17 children present with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of three staff records and interview, the center did not ensure one staff member's record was complete.

Evidence: 1) The references in the record for Staff #1, hired on 08/20/21, were obtained on 08/26/21 and 08/28/21. Each staff record should contain documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering. 2) During interview, a member of management acknowledged the references were not obtained for Staff #1 prior to hire.

Based on a review of three staff records and interview, the center did not ensure one staff member completed a national fingerprint check before the first day of employment.

Evidence: 1) The national fingerprint check in the record for Staff #3, hired on 09/04/20, were dated 01/10/20. 2) During interview, a member of management reported Staff #3 was a volunteer in January 2020, but stopped volunteering in March 2020. Staff #3 was hired on 09/04/20, but new fingerprints were not completed prior to the first day of employment.
