During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site code compliance inspection was initiated on 04/07/2025 and completed on 04/08/2025. The on-site inspection began at 10:05am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were 12 children present with three staff. The inspector reviewed five children?s records on-site. Five staff records were reviewed electronically on 04/08/2025. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliances with applicable code sections, and violations are documented on the violation notice issued to the program.

The center is required to have employees undergo a background check every five years.

The central registry check for Staff #2 and Staff #5 were overdue by more than eight months and the sworn statement for Staff #2 and Staff #5 were overdue by more than a year and seven months.
Plan of Correction: Staff will complete a new sworn statement and new central registry searches were requested. In the future, staff will repeat background checks every five years.
