Inspector Notes
An unannounced, on-site renewal inspection was initiated on 01/28/2025 and completed on 01/28/2025. The on-site inspection began at 2:50 p.m. and ended at 3:35 p.m. The inspector reviewed compliance in the areas listed above. There were 15 children present and 3 staff. The inspector reviewed 5 children?s records on-site on 01/28/2025 and 3 staff records and 5 agent/applicant records on-site at the administration office on 01/28/2025. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1The following non-medical emergency supplies shall be required: one working battery-operated radio in each building used by children.
A working battery-operated radio was not available during the inspection.
