Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews (if applicable) and a virtual tour of the program (if applicable).

A code compliance inspection was initiated on March 8, 2022 and concluded on March 10, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were twenty-eight children present, ranging in ages from infants to five years old, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of three children?s records and three staff records were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.

Based on review, facility did not ensure staff have required background check prior to employment for the state a staff has resided in within the past 5 years.

Evidence: The record for staff #3 (start date 1/31/2022) had documentation of living outside the state of Virginia within the past 5 years and there was not a criminal history search completed for 1 of 2 states noted as prior residences.
Plan of Correction: Staff researched the requirements for applying for the criminal history search during the inspection. Staff will have fingerprints completed and the criminal history search will be submitted.
