Inspector Notes

An unannounced inspection was initiated on 07/09/2024 and completed on 08/16/2024 in response to a complaint received by the licensing office on 06/28/2024 relating to allegations of staffing and supervision and special care provision and emergencoes. Two inspection were combine for this complaint RECDC and Subsidy and Health and Safety inspeciton. The inspector reviewed
records. This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid.

Though not a part of the original complaint, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on
August 23, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-790-560-A
Based on staff interview and observation, the vendor did not ensure A. Staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence:
Staff #2 did not have a TB test on July 9, 2024 . Staff #2 was counted in ratio on June 13, 2024 confirmed by director. When volunteer is counted in ratio they would be known as a staff member and need to have all required staff requirements.

Standard 8VAC20-790-730-I
Based on observation of video footage, the licensee did not ensure children less than 12 months of age while sleeping or resting in crib shall have stuffed toys.

Evidence :
1- There was a large duck block in the crib .
2- The duck block was supporting infant sitting up in the crib observed in video.
3- Confirmed with director during review of video footage.

Standard 8VAC20-790-820-A
Based on observation, the vendor did not ensure when children are placed on feeding table the protective belt was fastened securely.

Evidence: Video footage was reviewed, Staff #1 was observed put child #1 in the the bucket seat at the feeding table / activity table and did not fastened protective belt. Staff #1 came back after changing a child and fastened protective belt. Video footage was reviewed with Director.

Based on observation the licensee did not ensure, bottles shall not be used while the child is in his designated sleeping location.

Evidence:
In the infant room , infant was seen with a bottle sitting up in the crib . Confirmed by director when reviewing video footage.
