Inspector Notes

An unannounced inspection was initiated on 08/22/2025 and completed on 08/22/2025 in response to a complaint received by the licensing office on 08/07/2025 relating to allegations of physical plant and special care and provision and emergency. The inspector reviewed 1 child record. This inspection included document review, tour of the facility, interviews, observations and measurements.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Ensuring that all areas of the premises accessible to children are free of obvious injury hazards. There was a fence with a red metal covering weaved in the fence for privacy. The metal pieces were broken and had sharp edges in multiple area. There were large toys stacked on each other was a tipping hazard. The laminate floor is raised and detaching from floor causing a tripping hazard.
Plan of Correction: Areas where fence is broken will not be play areas until fixed.
Toys unstacked staff instructed on danger.
Floor will ne covered with rug until replaced which will be as soon as roof is completed.
