Inspector Notes

An unannounced complaint inspection was initiated and completed on 11/10/2022 in response to a complaint that was received by the licensing department on 10/26/2022 regarding rodents and the physical plant. On 11/7/2022, the inspector was on site from 12:00 AM to 12:50 PM. Interviews with staff were completed, observations were made in the center, and documentation were reviewed. The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details. Based on observation, interviews, and review of documentation, the center failed to ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1 Two staff interviewed stated that a mouse was seen both alive (running through the infant classroom) and dead (caught on a sticky trap in the classroom area. 2. Staff stated that the mice had been noticed at the center for over a month. 3. The center director confirmed that sticky traps placed around the center, when the children are not in care. Sticky traps are taken up prior to the arrival of the children. 4. The health department observed rodent feces and debris behind the freezer and near the compartment sink in the kitchen per the inspection report on 11/4/2022. A violation was cited by the health department.
