Inspector Notes
A complaint inspection was initiated on June 1, 2023, and concluded on June 28, 2023. A complaint was received by the department on May 26, 2023, and June 14, 2023, regarding allegations in the areas of ratio, physical plant, and food and nutrition. Based on the information gathered and documentation provided during the investigation the preponderance of evidence doesn't support the allegation regarding ratios or food and nutrition; however, the complaint was deemed valid with regards to physical plant. Violations were cited and can be found on the facility violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations
2Evidence:
1. The stairs outside leading up to the June Bug and Bumblebee classroom, had six nails that were protruding approximately 1 inch -2 inches from the stairs.
2. The director stated that she would contact the owner and have him hammer the nails down. The director stated that children will not use the stairs until the repairs have been made.
3. The wall in the kitchen next to the door, has a large hole in the wall about 5 ? - 6 inches stuffed with plastic bags.
4. The director said that she was aware of the hole.
Evidence:
1. A staff in the Joe?s classroom did not wash the infant?s hands after diapering. Staff removed the infant from the changing table and sat the infant on the carpet.
