Inspector Notes
An unannounced monitoring inspection was conducted on February 7, 2024. The inspection began at 10:50 AM and concluded at 12:40 PM. There were 69 children present with 12 staff supervising. Children range in age from 3 months to 5 years. The inspector observed children eating lunch. Lunch consisted of pizza, green beans, fruit, and milk. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies, and nutrition. A total of 6 children records, 6 staff records and 2 medications were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

3Evidence:
1. The record for staff # 4 did not contain documentation of a completed central registry background. Staff #4?s employment date was 7/31/2023.
Evidence:
1. The records for staff #2 (date of hire 1/29/2024) and staff # 3 (date of hire 1/29/2024), did not contain documentation of a negative tuberculosis test.
2. The director stated that both staff #2 and staff #3 completed a tuberculosis test but was unable to provide the documentation.
Evidence: The record for child #2 (enrollment date 5/15/2023), did not contain an allergy action.
