Inspector Notes

An unannounced monitoring inspection was conducted on February 7, 2024. The inspection began at 10:50 AM and concluded at 12:40 PM. There were 69 children present with 12 staff supervising. Children range in age from 3 months to 5 years. The inspector observed children eating lunch. Lunch consisted of pizza, green beans, fruit, and milk. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies, and nutrition. A total of 6 children records, 6 staff records and 2 medications were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-770-60-C-2
(Repeat Violation) Based on a review of staff records, the center failed to ensure that each staff record contained a central registry finding within 30 days of employment.
Evidence:
1. The record for staff # 4 did not contain documentation of a completed central registry background. Staff #4?s employment date was 7/31/2023.
Plan of Correction: Will email CPS, and put proof in file.
Standard 8VAC20-780-160-A
(Repeat Violation) Based on record review, the center did not ensure that documentation of negative tuberculosis (Tb) test for each employee met all requirements.
Evidence:
1. The records for staff #2 (date of hire 1/29/2024) and staff # 3 (date of hire 1/29/2024), did not contain documentation of a negative tuberculosis test.
2. The director stated that both staff #2 and staff #3 completed a tuberculosis test but was unable to provide the documentation.
Plan of Correction: Will follow up and send when completed.
Standard 8VAC20-780-60-A-8
Based on review of 6 children records, the center failed to obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: The record for child #2 (enrollment date 5/15/2023), did not contain an allergy action.
Plan of Correction: Did is obtaining and will send when received.
