1. On the preschool playground, there was a toy truck that was broken, exposing sharp edges.
2. A plastic playground structure located on the preschool playground, had a hole in the slide that exposed sharp edges that could snag clothing or skin.
3.The wood on the top step outside of the back door leading to the playground area, was chipped exposing a hole that could pose as a tripping hazard.
