Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated and concluded on 05/13/2021. The director was contacted by telephone to initiate the inspection. There were 62 children present and 12 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records, 3 staff records, and 3 applicant/agent records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on review of applicant background checks it was determined that not all background checks were repeated every five years. Evidence: The most recent central registry check on file for applicant A was dated 10/21/2015 and the most recent sworn statement form on file for applicant A was dated 09/10/2015.
Plan of Correction: The applicant will complete the central registry check and sworn statement and submit the results to licensing. Applicant background checks are completed at the business entity's location in Arizona. Applicant is not involved in supervision of children within this facility.
