Inspector Notes

A monitoring inspection was initiated and concluded on 04/28/2022. There were 107 children present, ranging in ages from 4 months to 5 years, with 16 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Based on observation of the facility it was determined that not all areas were maintained in safe and operable condition. Evidence: 1.) There were sharp plastic edges exposed on a play sink in the home living center of the Pre-K 1 classroom. 2.) There is rust exposed and water damage on the bottom of the door in the Pre-K 2 classroom. 3.) The fence surrounding the air conditioning unit on the playground is leaning into the fenced in area. The pole on the fence is rusted with sharp edges exposed. 4.) The bolts on the ground perimeter surrounding the swings has bolts protruding posing tripping hazard.
Plan of Correction: 1.) Administration removed the play sink during the inspection. 2.) The rust and water damage will be covered until it can be permanently repaired by maintenance. 3.) The area of the fence will be placed off-limits until it can be repaired by maintenance. 4.) This area will be placed off-limits until it can be repaired by maintenance.
Standard 8VAC20-780-440-A
Based on observation of the infant room it was determined that cribs were occupied by more than one child at a time. Evidence: Child A and Child B were observed sleeping in the same crib. It was reported that this was a parental request.
Plan of Correction: Child A and child B will sleep in their own cribs.
