When staff are supervising children, they shall always ensure their care, protection, and guidance.
On January 7, 2026, Staff #1 was using an iron to create a mask craft in an area accessible to children. Child #1 (age: 6 years) walked over to Staff #1, stood beside them and leaned forward to talk to another child. The child's face touched the iron, resulting in a second-degree burn that was approximately 2.5 inches in length. Staff #1 confirmed that they noticed a mark on the child's face shortly after the child moved away from the iron and offered an ice pack. When Child #1 was picked up by their parent, the child informed the parent that they had burned their cheek.

Plan of Correction: Staff will always ensure the care, protection and guidance of children. When planning and implementing any program activity, specific safety procedures will be developed and followed, such as ensuring items are maintained in a designated area not accessible to children.
