Inspector Notes

An unannounced renewal inspection was conducted on 4/18/2023 from 11:30 am-2:10 pm. There were 81 children supervised by 21 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, tabletop activity, and eating lunch. Areas of non-compliance are identified in the Violation Notice. Standard 22.1-289.035-A
Based on record review and interview, one of five staff background checks was not repeated every five years.

Evidence:
1. The most recent central registry result on file for Staff #2 is dated 1/31/2017.

2. The central registry results on file for Staff #2 expired on 1/31/2022.

3. Staff #2's new central registry application was mailed to the Virginia Department of Social Services on 2/15/2023, according to the documentation presented during the inspection.
Plan of Correction: Make sure when staff return from Leave of Absence to check file for any needed updates
Standard 22.1-289.058
Based on observation and interview, the center did not ensure that a building built before 2015 and serving preschool-age children is equipped with at least one carbon monoxide detector.

Evidence:
1. The building was built prior to 2015.

2. The center currently provides care for children who are preschool-age.

3. The administration confirmed that the center did not have a carbon monoxide detector and was unaware that one was required.
Plan of Correction: Maintenance has been contacted and scheduled to install carbon monoxide detector.

Based on record review and interview, one of five staff records did not have an orientation completed within 7 days of the date of assuming job responsibilities.

Evidence:
1. Staff #5 (DOH 6/7/2019) record did not have documentation of receiving an orientation of their current job responsibilities.

2. The administration stated that staff #5 did receive orientation, but they were unable to locate the documentation.
Plan of Correction: Document was located in file drawer the day of the inspection.

Based on record review and interview, one of five staff records was missing two references.

Evidence: Staff #3 (DOH: 4/17/2023) record was missing documentation that two or more references as to character and reputation as well as competency were checked before employment.
