During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on August 27, 2025 and completed on August 27, 2025, as a part of the current licensure period. The on-site inspection began at 10:15am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were 48 children present and 14 staff. The inspector reviewed 5 child files and 5 staff files on-site. This inspection included: document review, postings review, tour of the facility and playgrounds, interviews, and observations. Inspection Summary amended September 8, 2025.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on Sept. 12. A POC submitted after this date will not appear on the public website.
Violations
5An independent contractor, who did not meet staff qualifications and was not an employee of the center, was being included in ratio for approximately 45 minutes.
~Cots and child materials were being kept in the bathrooms (next to the toilet) in Classrooms D, E, and F, that are routinely used by the children.
~Cots were propped up in the corner of Classrooms D and F in preparation of naptime, which was over an hour ahead in the schedule. The schedule was confirmed by staff in these classrooms. This was a hazard for the children who could be hit by a falling cot.
~Bibs were observed hanging, within reach of the children, on the sides of four out of five cribs, in classroom A. Infants were napping in the cribs during this time.
Staff 4 was observed supervising five infants for a period of more than 5 minutes, while second staff was in the bathroom.
Staff #1 was observed on the playground (during FitKids class) supervising twelve children (youngest child 2 years old) from approximately 10:15am-11:00am. Individuals not directly employed by the center, do not count in ratio.
