Evidence:
1. Staff #1 was observed working alone in the Pre-K classroom with 11 children. The children present were ages 4 years old.
2. Staff #1 was observed working alone in the Pre-K classroom with 11 children. The children present were ages 4 years old.
3. Staff #4 confirmed the two classrooms were not operating with the correct staff-to-child ratio.
Plan of Correction: The facility responded: We thought we had been approved for the staff-to-child ratio waiver. The classrooms were adjusted to meet the correct ratio, and we have submitted the staff-to-child ratio waiver to be approved.
