Inspector Notes

A Subsidy Health and Safety Inspection (SHSI) was conducted on 05/10/2023 from 1:55pm to 2:45pm to review supplemental health and safety requirements There was 1 child age 15 years old (0 points) with the Provider. The programming area, attendance, outdoor playground area, menus, Provider record, 4 children?s records, emergency drills, emergency supplies, and policies were reviewed. Child was observed being dropped off by the school bus after school. There Provider had current certification in CPR and First Aid training.

A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found.
